TtCC referral form for professionals

Surrey Coalition of Disabled People and Surrey Action for Carers are running an initiative to supply isolated or lonely adults with a tablet computer (and where required SIM data) alongside technical support and access to supported online events and forums. This form is for third party referrals from statutory and partner agencies who consider that their client may benefit. Referral does not guarantee acceptance. We particularly seek referrals from disabled households, those with carers or a mental health diagnosis. For more information email, or call Pete Dommett 07471518998
