TtCC referral form for professionals

Surrey Coalition of Disabled People and Action for Carers Surrey are running a service to tackle digital exclusion in Surrey.  We are particularly focusing on people at the most risk of digital exclusion – those who are disabled, living with a long term health condition, people with a caring responsibility and people from BAME groups.  The service can help people with a device (on long term loan), a ‘match’ with a volunteer tech angel, handouts and guides, connection with a virtual social programme and opportunities to virtually volunteer.  Some people just use one part of the service (for example, they may already have a device but they need a Tech Angel to give them the digital literacy and confidence to use it), whilst others use all elements of the service. 

This form is for professionals to refer someone that you are working with.  At present, the service is available in Surrey Downs ICP and East Surrey (CRESH) ICP areas, with North West Surrey launching very soon.
